Indications
When used with a low-calorie diet, orlistat is approved for the management of obesity, including weight loss and weight maintenance. Orlistat is also recommended for lowering the chance of gaining weight after losing it before. Orlistat 60 mg is indicated for overweight & obese patients with an initial body mass index (BMI) of 25 kg/m2, and Orlistat 120 mg is indicated for obese patients with an initial body mass index (BMI) of 30 kg/m2 or 27 kg/m2 in the presence of other risk factors (such as hypertension, diabetes, dyslipidemia).
Pharmacology
The gastric lipase inhibitor orlistat is reversible. By establishing a covalent link with the active serine residue site of gastric and pancreatic lipase, it exerts its therapeutic activity in the lumen of the stomach and small intestine. Thus, dietary fat in the form of triglycerides cannot be hydrolyzed into absorbable free fatty acids and monoglycerides due to the inactivated enzymes. Due to the fact that undigested triglycerides are not absorbed, the consequent calorie deficit may help with weight management.
Dosage & Administration
- One 60 mg or 120 mg capsule taken three times daily with a main meal containing fat is the recommended dosage of orlistat (during or up to 1 hour after the meal).
- The patient should follow a low-calorie, nutritionally-balanced diet with about 30% of calories coming from fat.
- Three main meals should be consumed each day to evenly balance the daily intake of fat, carbohydrates, and protein.
- The dosage of Orlistat can be skipped if a meal is occasionally missed or doesn't have any fat.
- Patients should be advised to take a multivitamin containing fat-soluble vitamins in order to maintain proper nutrition because orlistat has been demonstrated to impair the absorption of several fat-soluble vitamins and beta-carotene.
- It is recommended to take the vitamin supplement at least two hours before or after taking orlistat, ideally before night.
Interaction
Iodine salts, levothyroxine, fat-soluble vitamins (A, D, E, and K), and beta-carotene absorption may be decreased. may lower amiodarone and ciclosporin plasma levels. may lessen the effectiveness of antiretrovirals, antidepressants, and antipsychotics, as well as antiepileptic medications (such as valproate and lamotrigine) (including lithium). additive impact of diabetes medications on glycaemic control. Possibly leading to hormonal contraceptive failure. may enhance warfarin's anticoagulant effects.
Contraindications
Orlistat is contraindicated in Pregnancy, Patients with chronic malabsorption syndrome, Patients with cholestasis, and Patients with known hypersensitivity to Orlistat or to any component of this product.

Pregnancy & Lactation

Precautions & Warnings
Patients should be advised to adhere to dietary guidelines. gastrointestinal events may increase when orlistat is taken with a diet high in fat (>30% of calories from fat). The daily intake of fat should be distributed over three main meals. If a meal is missed, the dose of orlistat may be omitted.
