Linagliptin
DPP-4 inhibitor: Prevents degradation of incretin hormones (GLP-1 and GIP).
Enhances glucose-dependent insulin secretion.
Reduces glucagon release from α-cells.
Improves blood glucose without causing hypoglycemia.
Metformin Hydrochloride
A biguanide class oral antidiabetic.

- Already on Metformin: 2.5 mg Linagliptin + current Metformin dose twice daily.
Extended Release Tablets
- Maximum Dose: 5 mg Linagliptin + 2000 mg Metformin once daily with meal.
Initial Dosing:
- Not on Metformin: 5 mg/1000 mg once daily with food.
- Already on Metformin: 5 mg Linagliptin + equivalent Metformin once daily.
- Switching from immediate release: Use the same total daily doses.
If using 2.5 mg Linagliptin + 1000 mg Metformin XR, take two tablets together once daily.
