Indications

Salbutamol Inhaler is used to treat or prevent bronchospasm in individuals with bronchial asthma as well as reversible airway obstruction in individuals with bronchitis and emphysema.

Exercise-Induced Bronchospasm: Salbutamol Inhaler can be used to treat acute dyspnea and prevent exercise-induced asthma or as a preventative measure before exercising.

Pharmacology

The 2-adrenoceptor is a selective agonist of salbutamol. It has therapeutic effects on the 1-adrenoceptors of cardiac muscle but little to no effect on the 2-adrenoceptors of bronchial smooth muscle. Salbutamol causes short-acting (4-6 hours) bronchodilation with a rapid onset in reversible airway obstruction (within 5 minutes). By acting as an anti-inflammatory on mast cells, it also prevents the release of bronchoconstrictor mediators like histamine, neutrophil chemotactic factor (NCF), and prostaglandin D2.

Dosage

For relief of acute episodes of bronchospasm:

• Adults: 1 or 2 puffs as necessary. The maximum dose is up to 8 puffs in 24 hours.
• Children: Half the adult dose.

To prevent allergen-or exercise-induced bronchospasm:

• Adults: 2 puffs 15 minutes prior to exercise or exposure to the allergen. The maximum dose is 2 puffs, up to 4 times a day.
• Children: Half the adult dose.

Chronic therapy:

• Adults: Up to 200 mcg (2 puffs) four times daily.
• Children: Up to 200 mcg (2 breaths) four times daily.

Administration 

• Remove the cap. Shake the inhaler vigorously once recently after each use (at least six times).
• Shake the inhaler well if it is new or if it hasn't been used in a week or more to ensure that it works.
• Then, inhale one puff into the air to confirm this.
• Hold the inhaler upright and exhale as deeply as you can while still feeling comfortable.
• Put the actuator near the lips, close to the mouthpiece, and between the teeth.
• Press down firmly and completely on the medicine canister while breathing deeply and gradually through your mouth.
• Take the inhaler out of your mouth. Continue holding your breath for at least 10 seconds, or however long feels comfortable.
• If the doctor has recommended taking more than one inhalation during the treatment, wait one minute in between each one (inward breaths).
• Repeat steps 4 through 7 after thoroughly shaking the inhaler. Replace the cap on the mouthpiece after use.

Interaction

Salbutamol and non-selective beta-blockers like propranolol should not be prescribed simultaneously in most cases. 2-agonist treatment has the potential to cause significant hypokalemia. Concomitant treatment with xanthine derivatives, steroids, diuretics, and hypoxia may intensify this impact, therefore use caution if you have acute severe asthma. In such cases, it is advised that serum potassium levels be monitored.

Contraindications

Salbutamol inhaler is contraindicated in patients with a history of hypersensitivity to salbutamol or any other components of Salbutamol inhaler.

Side Effect

Salbutamol is generally well tolerated. Few side effects have been observed during inhalation. There are tremors, anxiety, muscle cramps, headache, palpitation, a small compensatory increase in heart rate, cardiac arrhythmias (including atrial fibrillation, supraventricular tachycardia, and extrasystoles), tachycardia may occur in some patients. Mouth and throat irritation may occur with inhaled salbutamol

Pregnancy & Lactation

Pregnancy Classification C. In pregnant women, there are no sufficient and well-controlled studies using salbutamol inhalers or salbutamol sulfate. During pregnancy, this inhaler should only be used if the possible benefit outweighs the potential risk to the fetus. The components of the Salbutamol inhaler are not known to be excreted in human milk. When administering this inhaler to a nursing mother, exercise caution.

Precautions & Warnings

• Salbutamol sulfate inhalation can result in a potentially fatal condition called paradoxical bronchospasm.
• In the event of paradoxical bronchospasm, the Salbutamol inhaler should be stopped immediately and a different course of treatment should be taken.
• It's crucial to keep in mind that paradoxical bronchospasm frequently occurs following the first use of a new canister of inhaled formulations.
• Cardiovascular Effects: The salbutamol inhaler, like all other 2-adrenergic agonists, may have clinically significant cardiovascular effects in some users, such as changes in blood pressure or pulse rate. If such side effects occur, the salbutamol inhaler may need to be discontinued. Beta-agonists have also been connected to ECG abnormalities like ST-segment depression, QTc interval lengthening, and flattening of the T wave. These observations' clinical relevance has not yet been established. Salbutamol inhalers, like all sympathomimetic amines, should be used with caution in people who have underlying cardiovascular diseases, especially coronary insufficiency.
