Adult Dose:
Methotrexate Toxicity:
- Oral: 15 mg every 6 hours for 10 doses, starting 24 hours after the methotrexate infusion. Continue until methotrexate serum levels are <0.05 micromolar.
- Intravenous/Intramuscular: Administered as needed depending on the patient's condition.
Folate-Deficient Megaloblastic Anaemia:
- Oral: 15 mg daily.
- Intramuscular/Intravenous: Up to 1 mg/day depending on the severity of the condition.
Adjunct to Fluorouracil in Colorectal Cancer:
- Intravenous: 200 mg/m² BSA (Body Surface Area) by slow IV injection over at least 3 minutes, followed by 370 mg/m² fluorouracil by IV injection.
- Treatment Cycle: Given for 5 consecutive days, repeated at 28-day intervals for 2 courses, and subsequent cycles every 4-5 weeks if the patient recovers from toxic effects.

- Folinic Acid (Calcium Folinate) enters cells as 5-methyl tetrahydrofolate, which supplies the necessary cofactor for nucleotide synthesis, bypassing the blockade caused by methotrexate.
- It stabilizes the binding of 5-dUMP and thymidylate synthetase, which enhances the activity of fluorouracil and counteracts the effects of folic acid antagonists such as methotrexate.

- Fluorouracil: Folinic acid may increase the toxicity of fluorouracil when used together.
- Sulfamethoxazole-Trimethoprim: Folinic acid may reduce the efficacy of this antibiotic combination in the treatment of Pneumocystis carinii pneumonia, particularly in HIV/AIDS patients.

- Hypersensitivity to folinic acid or any other component of the product.
- Pernicious anaemia or other megaloblastic anaemias due to vitamin B12 deficiency.
