Adults
Hypertension (Immediate-release):
- Start: 80 mg orally every 8 hours
- Maintenance: 80–320 mg/day divided q8h
Hypertension (Extended-release):
- Start: 180 mg once daily (lower if elderly/small frame)
- Max: 480 mg/day
Angina:
- IR: 80–120 mg every 8 hours, up to 480 mg/day
- ER: 180–480 mg/day at bedtime
Arrhythmias:
- 240–480 mg/day in divided doses

- Increased bradycardia & conduction issues: Beta-blockers, digoxin, clonidine
- Increased verapamil levels: Protease inhibitors, cimetidine
- Decreased verapamil levels: Rifampin, phenobarbital
- Increased levels of other drugs: Doxorubicin, cyclosporine, statins, buspirone
- Alcohol: May increase its blood levels
- Potentially fatal: Combined use with amiodarone, dofetilide, dantrolene, etc.

- Marked bradycardia
- 2nd or 3rd degree AV block (without pacemaker)
