Indication
Flugal 200 belongs to a class of drugs known as antifungals. Fluconazole is used to treat vaginal candidiasis, mucosal candidiasis (as oropharyngeal candidiasis, oesophagitis, mouth, throat, vagina, and other parts of the body, including fingernails and toenails, by inhibiting fungus growth and candiduria), systemic candidiasis, and cryptococcal infections (including meningitis).
Pharmacology
Fluconazole is a triazole antifungal medication that works by inhibiting the ergosterol component of fungal cell membranes. It is effective against a wide range of fungal pathogens and can be taken orally. Absorption is rapid and extensive (>90%) after oral administration. Fasting and fed states have the same bioavailability. Fluconazole's bioavailability is unaffected by gastric pH. The plasma half-life is about 30 hours. Fluconazole has a low plasma protein binding rate of 11% and is widely distributed throughout the body. The majority of the Fluconazole dose is excreted unchanged in the urine, with the remainder of the dose eliminated as metabolites.
Dosage & Administration
Adults
- Fluconazole's daily dose is the same for oral and intravenous administration because oral absorption is rapid and nearly complete.
- Fluconazole is recommended for oropharyngeal candidiasis at a dose of 200 mg on the first day, followed by 100 mg once daily for 7-14 days.
- Fluconazole is recommended for esophageal candidiasis at a dose of 200 mg on the first day, followed by 100 mg once daily for 2-3 weeks.
- Systemic Candidiasis and Cryptococcal Meningitis: 400 mg twice daily, then 200 mg once daily.
- Initial treatment for cryptococcal meningitis is recommended to last 10 to 12 weeks.
- Infections of the urinary tract and peritonitis: 50 to 200 mg once daily.
- Prophylaxis in Bone Marrow Transplant Patients: 400 mg once daily.
- Acute or recurring vaginal candidiasis: 150 mg once daily.
Children
- Fluconazole is recommended for oropharyngeal candidiasis in children at a dose of 6 mg/kg on the first day, followed by 3 mg/kg once daily. To reduce the likelihood of relapse, treatment should be given for at least two weeks. Esophageal
- Candidiasis: Fluconazole is recommended for children at a dose of 6 mg/kg on the first day, followed by 3 mg/kg once daily.
- Initial therapy for cryptococcal meningitis should consist of 6 to 12 mg/kg daily. Doses of up to 12 mg/kg/day may be used for three weeks, with treatment for cryptococcal meningitis lasting 10 to 12 weeks.
Interaction
Concurrent use of cyclosporin or phenytoin with Fluconazole raises the plasma level of cyclosporin or phenytoin. Fluconazole and warfarin use together prolongs the prothrombin time. When combined with Fluconazole, the level of Rifampicin decreases.
Contraindications
Fluconazole is contraindicated for patients hypersensitive to this drug and with advanced liver disease.
Side Effects
Nausea, abdominal discomfort, diarrhea and flatulence, occasional abnormalities of liver enzymes; rarely rash; angioedema and anaphylaxis reported.
Pregnancy and Lactation
