• Management of chronic kidney disease (stages 3 and 4 with abnormal presence of albumin in the urine) associated with type 2 diabetes in adults

✔️ How to Use Firendia 20

Take Kerendia exactly as directed by your doctor. Swallow the tablet whole with a glass of water—do not crush or chew it. You can take it with or without food. Avoid grapefruit or grapefruit juice during treatment as it might lead to adverse effects.

✔️ Side Effects of Firendia 20

Common side effects include high potassium levels, low sodium levels, high uric acid levels, and low blood pressure, which may cause dizziness or fainting. If you experience these, consult your doctor.

✔️ Mode of Action:

Finerenone blocks aldosterone, a hormone that affects salt and water balance, thus reducing blood pressure and alleviating stress on the kidneys, which can help slow kidney damage in diabetes and chronic kidney disease.

✔️ Missed Dosage & Overdose Effects

Missed Dosage

If you miss a dose, take it as soon as you remember, but do not double up. If you suspect an overdose, contact your doctor or visit the nearest hospital immediately.

Overdose Effects

• Symptoms: The primary effect of overdose is likely to be hyperkalemia.
• Management: Discontinue Finerenone immediately. Standard treatments for hyperkalemia should be administered. Hemodialysis is not effective for removing Finerenone due to its high plasma protein binding.

✔️ Interaction

Disclose all current medications, including antifungals, anti-HIV drugs, antibiotics, and others, to avoid interactions. Be particularly careful with diuretics, potassium supplements, and medications affecting blood pressure or liver enzymes.

✔️ Contraindications

• Concomitant Use with Strong CYP3A4 Inhibitors: Avoid using Finerenone with strong inhibitors of CYP3A4, such as itraconazole, ketoconazole, and ritonavir.
• Adrenal Insufficiency: Do not use in patients with adrenal insufficiency or Addison’s disease.

✔️ Pregnancy & Lactation

Pregnancy: There is no sufficient data on the use of Kerendia during pregnancy to assess the risk of major birth defects or adverse outcomes. Animal studies showed developmental toxicity at levels much higher than human exposures, but the clinical relevance is unclear. Given the background risk of birth defects and miscarriage, pregnant women should only use Kerendia if needed and prescribed by a healthcare provider.

Lactation: No data on the presence of Finerenone in human milk, its effects on a breastfeeding infant, or milk production. Animal studies suggest that Finerenone is present in rat milk and may affect pups. Avoid breastfeeding during treatment and for 1 day after discontinuation.

✔️ Precautions & Warnings

• Hyperkalemia Risk: Monitor serum potassium levels and estimated glomerular filtration rate (eGFR) before starting and during treatment. Do not start if potassium is > 5.0 mEq/L. Regular monitoring is essential, particularly for patients at higher risk or those on medications that affect potassium levels.
