✅ Description:

Chronic Kidney Disease

is the progressive loss of kidney function over time, typically occurring over several years. Without treatment, the damage becomes permanent, leading to a buildup of toxins and excess fluid in the body, which can cause various complications. CKD is associated with a higher risk of premature death, reduced quality of life, and increased healthcare costs.

Causes and Risk Factors:

  • Hypertension: High blood pressure can damage kidney blood vessels.
  • Diabetes: High blood sugar levels can harm kidney function.
  • Heart Disease: Cardiovascular conditions can affect the kidneys.
  • Nephrotoxins: Harmful substances or medications that can damage the kidneys.
  • Hyperlipidemia: High levels of fats in the blood can contribute to kidney damage.
  • Polycystic Kidney Disease: A genetic disorder causing cysts in the kidneys.
  • Glomerulonephritis: Inflammation of the kidney’s filtering units.

Signs and Symptoms:

  • General fatigue, weakness, and malaise
  • Nausea and vomiting
  • Frequent urination, especially at night (nocturia)
  • Fluid retention leading to swelling (edema)
  • Shortness of breath
  • Loss of appetite
  • Confusion and lethargy
  • Skin changes like bruising, itching, or pallor
  • Bleeding tendencies
  • Bone pain and fractures

Complications:

  • Hypertension: Elevated blood pressure.
  • Impaired Cognitive Function: Difficulty with memory and concentration.
  • Uremic Encephalopathy: Brain dysfunction due to toxin buildup.
  • Peripheral Neuropathy: Nerve damage leading to pain or numbness in extremities.
  • Renal Osteodystrophy: Bone disease related to kidney failure.
  • Arthropathy and Bone Pain: Joint and bone discomfort.
  • Fluid Retention: This can lead to hypertension and pulmonary edema.

Treatment Options:

Medications:

  • ACE Inhibitors or ARBs: Help control blood pressure and protect kidney function.
  • Diuretics: Reduce fluid buildup.
  • Erythropoiesis Stimulating Agents: Address anemia.
  • Sodium Bicarbonate: Helps manage acid-base balance.

Renal Replacement Therapy:

  • Dialysis: Artificially removes waste products and excess fluid.
  • Kidney Transplantation: Replaces damaged kidneys with a healthy donor kidney.

Prognosis and Prevention:

  • CKD often progresses to end-stage renal disease (ESRD) if not detected early.
  • Dialysis carries higher morbidity and mortality risks.
  • Individuals with diabetes or hypertension should monitor kidney function closely.
  • Preventive measures include:
    • Consuming a kidney-friendly diet
    • Engaging in regular, moderate exercise

Investigations:

  • Urine Tests: Assess for protein and other abnormalities.
  • 24-Hour Urine Test: Measures kidney function over a full day.
  • Blood Tests: Includes CBC, BUN, and creatinine levels.
  • Kidney Ultrasound: Provides images to evaluate kidney structure.
  • Biopsy: Examines kidney tissue for specific diseases.

Relevant Products / প্রাসঙ্গিক পণ্য

✔️ Uses of Firendia 20
  • Management of chronic kidney disease (stages 3 and 4 with abnormal presence of albumin in the urine) associated with type 2 diabetes in adults
✔️ How to Use Firendia 20

Take Kerendia exactly as directed by your doctor. Swallow the tablet whole with a glass of water—do not crush or chew it. You can take it with or without food. Avoid grapefruit or grapefruit juice during treatment as it might lead to adverse effects.

✔️ Side Effects of Firendia 20

Common side effects include high potassium levels, low sodium levels, high uric acid levels, and low blood pressure, which may cause dizziness or fainting. If you experience these, consult your doctor.

✔️ Mode of Action:

Finerenone blocks aldosterone, a hormone that affects salt and water balance, thus reducing blood pressure and alleviating stress on the kidneys, which can help slow kidney damage in diabetes and chronic kidney disease.

✔️ Missed Dosage & Overdose Effects

Missed Dosage

If you miss a dose, take it as soon as you remember, but do not double up. If you suspect an overdose, contact your doctor or visit the nearest hospital immediately.

Overdose Effects

  • Symptoms: The primary effect of overdose is likely to be hyperkalemia.
  • Management: Discontinue Finerenone immediately. Standard treatments for hyperkalemia should be administered. Hemodialysis is not effective for removing Finerenone due to its high plasma protein binding.
✔️ Interaction

Disclose all current medications, including antifungals, anti-HIV drugs, antibiotics, and others, to avoid interactions. Be particularly careful with diuretics, potassium supplements, and medications affecting blood pressure or liver enzymes.

✔️ Contraindications
  • Concomitant Use with Strong CYP3A4 Inhibitors: Avoid using Finerenone with strong inhibitors of CYP3A4, such as itraconazole, ketoconazole, and ritonavir.
  • Adrenal Insufficiency: Do not use in patients with adrenal insufficiency or Addison’s disease.
✔️ Pregnancy & Lactation

Pregnancy: There is no sufficient data on the use of Kerendia during pregnancy to assess the risk of major birth defects or adverse outcomes. Animal studies showed developmental toxicity at levels much higher than human exposures, but the clinical relevance is unclear. Given the background risk of birth defects and miscarriage, pregnant women should only use Kerendia if needed and prescribed by a healthcare provider.

Lactation: No data on the presence of Finerenone in human milk, its effects on a breastfeeding infant, or milk production. Animal studies suggest that Finerenone is present in rat milk and may affect pups. Avoid breastfeeding during treatment and for 1 day after discontinuation.

✔️ Precautions & Warnings
  • Hyperkalemia Risk: Monitor serum potassium levels and estimated glomerular filtration rate (eGFR) before starting and during treatment. Do not start if potassium is > 5.0 mEq/L. Regular monitoring is essential, particularly for patients at higher risk or those on medications that affect potassium levels.
✔️ Use in Special Populations
  • Pediatric Use: Safety and efficacy in individuals under 18 years of age have not been established.
  • Geriatric Use: No overall safety or efficacy differences were observed between older and younger patients in clinical trials. No dose adjustment is necessary for older patients.
  • Hepatic Impairment: Avoid use in severe hepatic impairment (Child-Pugh Class C). For mild or moderate hepatic impairment (Child-Pugh Class A or B), no dosage adjustment is required, but increased monitoring of potassium levels is recommended.
✔️ Storage:

Store Kerendia tablet in a cool and dry place, protected from direct sunlight, moisture and heat. Keep it away from children and pets. Discard any unused, expired medicine properly. Do not flush the medicine.
